We would require the following in a claim where we are appointed to determine the possibility that the insured driver was driving under the influence (DUI):
- Mandate form
- Claim form (including contact details of the insured driver)
- Verification report (if a verification has already been done)
- Any medical records
Keep in mind that the Medical Record Review and Analysis report is in support of other verifications done
i.e. scene investigation, liaison with police & traffic department, interviewing witnesses, tracking of the insured driver movements prior to an accident, etc.
